Two forms, the GE Course Substitution/Exception Form and the Substitution for Major or Support Courses Form, were revised in December 2004.  In addition, a new form, the GE Course Substitution for Study Abroad Form, was created to assist students taking courses abroad.  Changes on these forms were developed after over a year of discussion and input from department chairs/heads, deans and associate deans, faculty, advisors, and administrative staff.

We ask that all department offices discard the old forms and use the new ones, in order to provide consistency on campus and minimize confusion for students.  What follows is some further information about the forms and where to obtain them:

These forms are printed by the Records Office and have been distributed to the College Advisors. If you would like for students to be able to obtain these forms, please send them to the COB College Advising Office. The GE Course Substitution for Study Abroad forms are new forms designed for students wishing to receive Cal Poly credit for courses taken abroad.  Please advise students that these forms are available in the Study Abroad Office (building 38, room 106).  These forms are not distributed by the colleges or departments.
